Description - Warwickshire CC is seeking to commission a single provider to provide a short term targeted support service for vulnerable adults, aged 18 years and over, who do not meet the threshold to access statutory support from social care under the Care Act 2 014 or support from other agencies according to their specific eligibility criteria, but who the Council reasonably considers would benefit from short term targeted intervention to prevent, reduce and/or delay the need for longer term care and support and whose independence and well-being is, or may be otherwise vulnerable to significant harm. Preventing their needs from developing or deteriorating.
