Local Councils and the NHS are working together more effectively to meet people’s needs, for example by making sure that health and care servi ces are coordinated when someone leaves hospital, so they get the help they need to recover back in their own home.Suffolk County Council is undertaking Market Engagement prior to conducting a procurement exercise for the award of a contract to provide a supported discharge service for Adult and Community service and NHS.This service is not for domiciliary home care as no regulated activities will be required as part of the service delivery.
