REQUEST FOR QUOTATION (RFQ) for Circuits for Vyaire Ventilators LTV2T2200 Mongolia, EpiC Project Issue Date: 18 May 2023 (Thursday) Response Deadline: 30 May 2023 (Tuesday) FHI 360, a non-profit organization based in Washington DC, is requesting quotes from authorized supplier of Vyaire to provide circuits that are compatible to Vyaire Ventilator LTV2TM2200 as per particulars stated below. The circuits will be donated to the Ministry of Health (MOH), Mongolia under the USAID-funded EpiC project. S. No. Particulars Required Quantity 29657-001 ASSY PAT CIR W/O PEEP ADULT SPU 225 boxes 2 29658-001 - ASSY PAT CIR W/O PEEP PED SPU 50 boxes Requirement: Authorized distributors of Vyaire equipment and consumables who are based in countries near Mongolia (e.g., India, China, Kazakhstan, Philippines, Thailand, and others) are encouraged to respond with their quotations. Agencies that apply must have a export license and should be familiar with the documentation to support end-to-end delivery up to the consignee. Quotation to include: Unit quotes to be sent in USD against the product specifications above Packaging, insurance, and shipment cost (by air and sea) to be specified clearly Customs clearance and other taxes (as applicable) Expected time of delivery from the date of issuing the Purchase Order Payment terms to be stated. CONTRACT MECHANISM FHI 360 expects to award a purchase order to the vendor. RESERVED RIGHTS All RFQ responses become the property of FHI 360, who reserves the right in its sole discretion to: To disqualify any offer based on offeror failure to follow solicitation instructions. FHI 360 reserves the right to waive any deviations by offerors from the requirements of this solicitation that in FHI 360's opinion are considered not to be material defects requiring rejection or disqualification; or where such a waiver will promote increased competition. Extend the time for submission of all RFQ responses after notification to all offerors. Terminate or modify the RFQ process at any time and re-issue the RFQ to whomever FHI 360 deems appropriate. FHI 360 reserves the right to issue an award based on the initial evaluation of offers without discussion. Award only part of the activities in the solicitation or issue multiple awards based on solicitation activities. FHI 360 will not compensate vendors for preparation of their response to this RFQ. Issuing this RFQ is not a guarantee that FHI 360 will award a purchase order. FHI 360 may choose to award a purchase order to more than one vendor for specific parts of the activities in the RFQ.
