Description - Provision of the Health Visting and School Nursing Services (0-25 years with SEND) delivering the Healthy Child Programme (HCP). Health Visitors specialise in working with families and children aged 0 to 5 years to identify needs as early as possible and improve health and wellbeing by promoting health, preventing ill health and reducing health inequalities and lead the 0 to 5 years elements of the Healthy Child Programme.
