Description - Description: The Council requires a Managed Service for the Completion of Social Care Reviews, predominantly 6-8 week reviews, although it is envisaged that the Provider may be required to undertake other reviews, prior to and after 6 to 8 weeks. Rev iews will be undertaken to a standard which is 2014 Care Act compliant. This may be the first opportunity to review a newly implemented package of support.The Review allows opportunity for a review of outcomes, both in terms of the service offer to sustain and support people to live as independently as possible, and to make the most effective use of what can be provided locally to support people’s independence, where that is appropriate.
