Description - The New York Army National Guard has a requirement for Laboratory Services. To provide all labor, materials, and services to perform the requirements as stated in the attached Performance Work Statement. Utilize the incorporated Estimated Test Per Lo cation Breakdown sheet to formulate your quote. Please include per location prices with the estimated tests per location and the cost to complete all the estimated test for the entire base year. Also include a minimum of 1 walk in clinic address for each JOA region. Please complete and submit one sheet for the base year, option year 1, and option year 2. This sheet is incorporated in the RFQ and is included in the SAM. gov posting for convivence.
