Description - To provide specialist assessment of veterans' social care needs across the South East. This will include provision of social care coordination and intensive support to veterans experiencing social care crisis whereby the individual's social needs are interfering with their ability to engage with an appropriate mental health pathway. It will require supporting veterans with issues around employment, housing, debt / welfare, disability, social inclusion and prescribing at the earliest opportunity following case allocation when clinically appropriate to do so.
