Description - Description: As part of the deployment of early and joint discharge planning (PPCC), an MSSS project, the CISSS des Laurentides wishes to acquire a computerized collaborative tool to facilitate communications between the different stakeholders involv ed in hospital discharges. Patient discharge planning is increasingly complex and requires the intervention of several stakeholders in order to make discharge safe.The tool must at least allow stakeholders to: - Find in the same place, at any time, and regardless of the workplace, all the information related to PPCC: the tool does not, however, in any way replace the user's clinical file; - Become aware of current follow-ups, requested follow-ups
